☑︎Complete the "STOKES Student and Contact Info" Form in Genesis.

☑︎Complete the Health Record and Medical Release document. This form must be notarized before being submitted to LCJSMS. Click here for information about the notary at LCJSMS.

☑︎Submit the trip fee. Online payment is available in Genesis under the "Fees and Fines" tab. Checks may be submitted to LCJSMS, payable to the "LCJSMS Activities Fund". The cost of the trip is $150 ($75 for students with free/reduced lunch).
